Three things that make you eligible
If all three describe you, you can almost certainly enroll — and we'll confirm your coverage before you do.
You have Original Medicare
You're enrolled in Original Medicare — Part A and Part B (also called Traditional or Fee-for-Service Medicare).
You have a covered condition
You're living with high blood pressure, diabetes or prediabetes, chronic musculoskeletal pain, or depression.
You live in the United States

The ACCESS program is for people with Original (Traditional) Medicare. If you're on a Medicare Advantage (Part C) plan, you're not currently eligible — but reach out and we'll let you know your options.
This program is for you if…
- You have Original Medicare — Part A and Part B
- You also hear it called Traditional or Fee-for-Service Medicare
- You have high blood pressure, diabetes, chronic pain, or depression
- You want extra day-to-day support between doctor visits
Not eligible right now if…
- You're enrolled in a Medicare Advantage (Part C) plan
- You only have Medicare Part A (hospital) without Part B
- You don't currently have one of the four covered conditions
